HIP appointments are focused on helping you develop practical tools and solutions that can be used straight away.
Rachel, Health Coach works alongside patients to support positive health and lifestyle changes. She can help with setting and achieving health goals, managing long-term conditions, improving nutrition and physical activity, navigating health services, understanding treatment plans, and building confidence to take charge of your own health and wellbeing. Health coaching is patient-led and tailored to your individual needs and goals.
These services are available free of charge to enrolled patients and can be accessed through a referral from your healthcare team. Together, Margaret and Rachel provide valuable support to help you improve both your physical and mental wellbeing.
